I lost my Holyangel Gran MAster Armor at +11 while using TOCA !

Issue: Lost item when upgrading via chaos machine with Toca,  Holyangel Gran MAster Armor  from +10 to +11. Dont know what is the issue, but i did register my toca when doing the upgrade, somehow item disappeared.

By somehow, this can happened very rare. I see by logs, that you've used TOCA in this mix, but after fail, the item was destroyed. 

The armor have restored to the your vault.
